​Weight Assessment and Documentation of Counseling for Nutrition and Physical Activity for Children and Adolescents

Percentage of patients 3-16 years of age who had their height, weight, and BMI documented.

Childhood Immunization Status Including Children Receiving two Hepatitis A and two Influenza Vaccines Before their Second Birthday

Percentage of children 2 years of age who had the following vaccines by their second birthday: four DTaP; three IPV; one MMR; two HiB; three Hep B; one VZV; four PCV; two Hep A; two or three RV; and two flu.

TestIng for Children with Pharyngitis



Percentage of encounters that involved patients who were 3-18 years old at the beginning of the reporting period and were diagnosed with pharyngitis. To qualify, patients must have been prescribed an antibiotic and received a group A streptococcus (strep) test for the episode.

Maternal Depression Screening

Percentage of mothers were who screened for postpartum depression within six months of a child's birth.

Smoking Status Recorded



Percentage of unique patients whose current smoking status is documented in the medical record.

Influenza Immunization



Percentage of patients who received an influenza immunization during the flu season. Only patients who were 6 months or older at the beginning of the reporting period are counted.
